Position: Full Time CAI Life Skills Coach
Reports to: Family Support Manager
JOB SUMMARY:
Follows daily activities which facilitate individual learning experiences that assist in the development of living skills consistent with the participant’s objectives identified in the Individual Support Plan (ISP)/ Plan of Care (POC) and with the program design. Helps individuals build relationships and make life lasting connections in the community. Works collaboratively with ESWG’s administration and other staff to ensure all aspects of the program development and delivery are consistent with ESWG’s mission.
DUTIES AND RESPONSIBILITIES:
Follows the implementation of ISP/POC to include objectives in areas of daily living skills (ADLs).
Assists in participant assessments, planning and evaluation processes. Attends ISP/POC meetings. Assists participant in the development and implementation of his/her support plan.
Works with the Adult Director, Program Director, and Assessment Specialist to promote, plan, organize and provide activities, which enhance learning experiences for each adult.
Follows a weekly schedule of community outings according to ISP/POC goals and objectives.
Facilitates the participation of clients in typical and integrated activities, events and organizations in the clients’ neighborhood or local community.
Supervises activities to ensure safety at all times following established safety standards.
Provides therapeutic care to maintain or restore functional abilities necessary for personal independence.
Provides personal attendant care as necessary.
Maintains professionalism while working outside of the Easterseals’ facility.
Fosters connections between the clients and persons without disabilities who are not paid to be with the person.
Transports participants to community outings.
Attends in-service training to maintain and increase knowledge and skills in the field of adult day services and special education. Successfully completes at least 20 hours of relevant training per year. Maintains current First Aid, CPR, fire safety, and CPI certifications.
Maintains all assigned documentation and participant records. Tracks changes relevant to goals and objectives.
Adheres to ESWG's policies and procedures, and appropriate licensing and accreditation standards.
Maintains effective and co-operative work relations with others.
Performs other duties as assigned by the Directors.
WORKING EXPECTATIONS:
Staff will exercise professionalism in the presence of family, community, and staff.
Staff will ensure that all activities and conversations are always geared toward what is appropriate.
To ensure quality of services provided, staff are expected to receive a rating of 20 (meets expectation) or higher on all performance reviews. Scoring under 20 will result in the staff member being placed on a 1-3 month probation until the level of quality is improved or termination may result.
QUALIFICATIONS:
High School Diploma or GED required.
1 year of related experience working with individuals with disabilities required.
Possess good interpersonal and organizational skills.
Ability to communicate well orally and in writing.
Ability to interpret and carry out written and oral instructions.
Obtain fingerprint criminal background check clearance.
